Order Optimizer support for delivery surcharges

Add Order Optimizer support for including delivery surcharges in calculating the estimated shipping cost for options considered by Optimizer.

Surcharges include but are not necessarily limited to:
1. Delivery Area Surcharges (DAS)
2. Extended Area Surcharges (EAS)
3. Remote Area Surcharges (RAS) - e.g. AK/HI
4. Fuel Surcharge (FS)
5. Residential Surcharge (RS)
6. Hazardous Surcharge (HS)

#1 and #2 and #3 (DAS/EAS/RAS) are effective by destination zip code (5 digits). DAS, EAS, and RAS are each fixed dollar amounts per package for all zip codes indicated as DAS or EAS or RAS (these are each applied as separate surcharge amounts). DAS/EAS/RAS zip codes can vary by carrier; some carriers may not have DAS/EAS/RAS. Dollar amounts for DAS, EAS, and RAS also vary by carrier. EAS is a higher amount than DAS; RAS is higher than EAS.

#4 (FS) changes often based on a government fuel price index. The FS is a fixed percentage added to the shipping rate. The FS can vary by carrier and mode (e.g. jet fuel has a different index & surcharge than diesel fuel). It may be possible (and desirable) to ingest this public index and automatically adjust the FS configurations in Order Optimizer. Not all carriers have FS.

#5 (RS) is an additional charge added to packages delivered to residential addresses. Since most orders are delivered residential Optimizer can reasonably assume all orders for a given carrier include this charge. It would however be nice to have a separate field to maintain this value vs. having to manually add it to the existing standard rates in today's configurations. Not all carriers have RS.

#6 (HS) is an additional charge for shipments containing certain hazardous materials - this feature should be considered as a future phase along with RFE #103898 ('Ship Alone' enhancement to incorporate item attribute based packaging logic).

After this enhancement a given order's estimated shipping charge could potentially include base shipping plus DAS/EAS/RAS plus RS plus FS plus HS.
